I'm not able to add it in a custom location

If you're attempting to use the shoutbox in a location other than the predefined, you'll need to set the automatic display to disabled and fill in both the script and template fields.
And obviously if the template name is wrong, it'll not show up.

Can you be a bit more specific about what values you attempted to use?

I added the following

Shoutbox Deployment : index
Manually Display Shoutbox : FORUMHOME

Auto-Display Shoutbox : Disabled

Added {vb:raw vbshout} to FORUMHOME

Same here, just upgraded to 4.5.0 but it doesn't show up.
have these settings:

Active: Yes
Auto Display: Disabled
Shoutbox Deployment: index,vbulc
Manually Deploy Shoutbox: FORUMHOME,ulc

And added {vb:raw vbshout} to the templates FORUMHOME and ulc where I want the shoutbox to display.

This has been fixed in the upcoming v4.5.1 maintenance release.
